All About Cracks, Fractures, and Open Apex

May 03-04 2025
Timing: 10:00 – 17:00

The master class is conducted in English with simultaneous translation into Russian

The endodontics course includes a review of modern methods of canal instrumentation and obturation, covering both steel and nickel-titanium instruments, clinical protocols, and addressing errors in the treatment process.

About course

The endodontics course covers everything related to cracks, fractures, and open apex. The program addresses issues such as Cracked Tooth Syndrome (cracks and fractures in the crown and root portions of the tooth), the etiology and pathogenesis of vertical root fractures, nuances of apexification and apexogenesis, as well as modern methods of revascularization and revitalization or regeneration. Participants will gain a comprehensive overview of clinical symptoms, diagnosis, treatment methods, prognosis, as well as preventive recommendations and a comparative review of various treatment options.

Course program

Cracked Tooth Syndrome

  • Definition, etiology, and pathogenesis.
  • Classification (Craze Lines – crack, Fractured cusp – broken cusp, Cracked tooth – fractured tooth, Split tooth – split tooth).
  • Clinical signs.
  • Diagnosis.
  • Treatment methods and prognosis.
  • Recommendations for prevention.
Vertical Root Fracture

  • Etiology and pathogenesis.
  • Role of pathology in treatment prognosis.
  • Clinical and radiographic signs.
  • Cone-beam CT and VRF.
  • VRF and failure of endodontic treatment.
  • Clinical recommendations for endodontists to reduce the likelihood of vertical fractures.
  • Biomechanics of canal instrumentation.
  • Chemical impact on physical structure.
  • Mechanical aspects of lateral compaction and vertical condensation.
  • Clinical recommendations for orthopedists to reduce the likelihood of vertical fractures.
  • Issues with „reinforcement”.
  • Choice of channels for anchor pins/posts.
  • Orthopedic canal treatment.
  • Step-by-step diagnostic process (including exploratory surgery and primary treatment).
  • Modern approach to problem-solving.

Apexification and Apexogenesis

  • Embryology and root formation (the concept of Hertwig’s epithelial root sheath – HERS).
  • Procedure choice: apexogenesis or apexification.
  • Nuances of the apexogenesis procedure. Apexogenesis and direct pulp capping.
  • Apexification in the practice of a pediatric dentist (childhood) and an endodontist therapist.
  • Open apex options.
  • Unfinished development.
  • Apical resorption.
  • Materials science: Ca(OH)2, MTA, IRM, SuperEBA.
  • From multi-stage procedures to single-visit techniques.
  • Biological rationale.
  • Technical execution.
  • Author’s methodology for case selection (not as simple as desired).
  • Choice of treatment methods and their comparative characteristics in young and adult patients.
  • Dangers and their prevention.
  • Possible restorations.

Revascularization, Revitalization, or Regeneration?

  • Embryology news as the basis for biotechnological breakthroughs. Stem cells of the apical papilla.
  • Revascularization – a reality of clinical practice or just an illusion?
  • Technique execution and analysis of clinical examples.
  • Tricalcium silicate-based materials (MTA, Biodentine).
  • Tricalcium silicate materials in clinical practice: indications, properties, handling, and limitations.
